- Comprehensive health information
- Laboratory or diagnostic test results
We collect your personal information in order to:
- Provide a comprehensive service and safe treatment
Besides our staff, we share this information with:
- Your primary provider such as your General Practitioner (GP) or other health professional directly involved in your care, which may include referrals to any appropriate health service or agency with your informed consent. In an event of a mental health emergency, we have a duty of care to contact the Southland Mental Health Emergency Team or the Police if there is immediate or imminent risk to yourself or others.
Providing some information is optional. If you choose not to provide pertinent health information, we will be unable to provide treatment or services.
We keep your information safe by storing it in a secure and encrypted patient management system or within your own GP practice. Only those directly related to your care will have access to your information unless you have provided consent in writing.
We keep your information for the duration of the care episode then we will transfer all information to your primary provider at which point we will securely erase all digital information and/or use a secure document destruction service.
You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. HealthOne is a south island based secure electronic record that allows registered healthcare providers directly involved in your healthcare, to quickly access information such as your test results, allergies, medications, GP summaries and hospital information. HealthOne strictly adheres to the principles of the Privacy Act, 2020 as well as Rule 5 (Storage and Security of Health Information) set out in the Health Information Privacy Code 2020. Access is only possible via an approved highly secure healthcare information network which is regularly audited and tested.
